SUBMISSION OF CAPITAL STRUCTURE AND UPDATED LIST OF STOCKHOLDERS, DIRECTORS AND OFFICERS
For the purpose of updating the records of this Commission, all insurance companies and insurance and reinsurance brokers are hereby required to submit on or before June 30, 1997 and every year thereafter, together with the application for renewal of license:
1. a certification by the Corporate Secretary on the capital structure of the company indicating the name of stockholders, nationality, number of shares owned and amount paid.
2. a list of current directors and officers.
