DATA ENTRY TO INFO SHEET
It has been noted that truthfulness in entering workers' personal and employment data in the Info Sheet has not been faithfully observed in several instances. Realizing the repercussion of false statements/misrepresentation when worker claims arise, veracity of the information provided in the Info Sheet by the agency for contract processing is of vital importance.
In view thereof, the following additional statements shall be adapted to the Info Sheet to ensure the veracity of the information entered thereto to be signed by the President of the agency or his authorized representative:
