[ PRC RESOLUTION NO. 2003-159A, SERIES 2003, December 01, 2003 ]

AMENDING ITEM II (B) OF PRC RESOLUTION NO. 2002-76, SERIES OF 2002 REQUIRING APPLICANTS IN LICENSURE EXAMINATION TO SUBMIT CERTIFICATES OF LIVE BIRTHS PRINTED ON NATIONAL STATISTICS OFFICE (NSO) SECURITY PAPERS



WHEREAS, the Commission issued Resolution No. 2002-76 series of 2002 dated November 29, 2002 providing for the "Revised Guidelines and Procedures in Filing of Applications for Licensure Examinations" stating among others the documents to accompany an application for licensure examination.

WHEREAS, Item II (b) of said Resolution states that one of the documents to accompany an application is the Original Certificate of Live Birth issued by the Local Civil Registrar (LCR) with Registration Number indicated or copy of authenticated Birth Certificate by the National Statistics Office (NSO).

WHEREAS, there are Certificates of Live Birth issued by the Local Civil Registrar (LCR) of which were found to be spurious or fraudulent.

WHEREAS, to further establish the authenticity of the Birth Certificates submitted by applicants and registered professionals, the Commission entered into a Memorandum of Agreement with the National Statistics Office to provide assistance to PRC clients applying for licensure examinations.

WHEREAS, the NSO also has the objective of providing faster and more reliable service to the public at large.

WHEREFORE, the Commission RESOLVES as it hereby RESOLVED to amend Item II (b) of Resolution No. 2002-76 series of 2002 to read as follows:

"(b)

b.1) Original and Photocopy of the Certificates of Live Birth printed on National Statistics Office (NSO) security paper and signed by Ms. Carmelita N. Ericta with attached original receipt.

b.2) Original and Photocopy of the computer generated NSO Certificates of Live Birth stating at the dorsal portion thereof, "A Certified True Copy: Office of the Civil Registrar General" duly signed by an authorized NSO statistician in security paper with attached original receipt.

This Resolution shall take effect upon its approval and completion of publication in Official Gazette or in a newspaper of general circulation in the Philippines, whichever is earlier.

Let copies of this Resolution be furnished all schools, colleges or universities, public and private, offering courses for licensure examinations and accredited professional organizations for their information and guidance.
