[ DDB BOARD REGULATION NO. 3, s. 1988, July 22, 1988 ]

AMENDING BOARD REGULATION NO. 2, S. 1973 ON RECORDS AND REPORTS REQUIRED OF PHARMACISTS



Pursuant to the powers vested in the Dangerous Drugs Board under Section 36(a) of R.A. 6425, as amended, Sections 3 and 4 of Board Regulation No. 2, S. 1973 shall be amended to read as follows:

SECTION 1.       Records called for in the 2nd paragraph of Section 25(a) of the Law aforecited shall be accomplished in DDB Form No. 14-73 and shall be submitted to the Dangerous Drugs Board by hospital pharmacists dealing in dangerous drugs dispensed in the In-patient wards of hospitals and other similar institutions within fifteen (15) days following the last day of June and December of each year, copy furnished the City or Municipal Health Officer concerned.

SECTION 2.       All pharmacists dealing in dangerous drugs other than in hospitals and similar institutions shall forward to the Dangerous Drugs Board, certified true copies of records covering a period of six (6) calendar months, within fifteen (15) days following the last day of June and December of each year, copy furnished the City or Municipal Health Officer concerned.

SECTION 3.       Failure to comply with the provisions of Sections 1 and 2 shall be penalized with immediate revocation of the S-license issued to the pharmacist concerned and non-issuance of another S-license to the said pharmacist for a period of two (2) successive years.

SECTION 4.       All regulations inconsistent herewith are hereby repealed or modified accordingly.

SECTION 5.       Should any provision or provisions of this Regulation be declared unconstitutional, the remaining provisions shall nevertheless remain enforceable.

SECTION 6.       This Regulation shall take effect upon its approval.
