The Government of Malaysia informed this Administration of the appointment of eleven (11) medical clinics to conduct the pre-employment medical services of OFWs bound to their country. They are the following:

Pursuant thereto, concerned agencies and entities are advised to refer their applicants to any of the above-mentioned clinics for the requisite medical examination subject to compliance with existing rules and regulations.

Attached is a copy of the medical form (to be reproduced) which the Malaysian Government wants accomplished properly by the concerned medical clinics in the conduct of the medical examination in lieu of the usual form formerly issued by the clinics. The use of this form takes effect May 15, 1996.
