[ PRC RESOLUTION NO. 04-253 (A), S. 2004, December 09, 2004 ]

FURTHER AMENDING ITEM II (B) OF RESOLUTION NO. 2003-159(A) DATED DECEMBER 1, 2003 ON NEGATIVE CERTIFICATION OF RECORD OF BIRTH BY THE NATIONAL STATISTICS OFFICE (NSO)



WHEREAS, in Resolution No. 04-245 series of 2004 dated October 29, 2004, the Commission resolved to amend Item II (b) of Resolution No. 2003-159 (A) dated December 1, 2003 to lessen the problem of applicant in licensure examinations whose names do not appear in the NSO database, and are only issued a negative certification of no record of birth by the NSO.

WHEREAS, it has come to the attention of the Commission that not all Local Civil Registrars issue Certificate of Live Birth on security paper.

WHEREAS, in our visits to the regions we determined that many local civil registrars do not yet have the security paper, forcing the applicants to go back and ask them to send their files to NSO for the latter to issue a new certification based on the local civil registrar submission, thus defeating the purpose of Resolution No. 04-245 dated October 29, 2004.

WHEREFORE, the Commission RESOLVES, as it hereby RESOLVED to further amend Item II (b) of Resolution No. 2003-159 (A) dated December 1, 2003, which shall now read as follows:

    "b.1) Original and photocopy of the Certificates of Live Birth printed on National Statistics Office (NSO) security paper and signed by Ms. Carmelita N. Ericta with attached original receipt.

    b.2) Original and Photocopy of the computer generated NSO Certificates of Live Birth stating at the dorsal portion thereof, "A Certified True Copy: Office of the Civil Registrar General" signed by an authorized NSO statistician in security paper with original receipt.

    b.3) In case of a negative certification of no record of birth is issued by the NSO, the applicant shall have the Certificate of Live Birth issued by the Local Civil Registrar, whether on security or ordinary paper, currently authenticated by authorized NSO statistician. The currently authenticated Certificate of Live Birth, attached to the NSO Certification on negative certification, shall be submitted by the applicant in applying for the licensure examination."

Let a copy of this resolution be published in the Official Gazette or newspaper of general circulation, and furnished the UP Law Center, the Commission on Higher Education (CHED), all schools, colleges or universities offering courses with licensure examinations.

This Resolution shall take effect upon its approval.
