[ POEA MEMORANDUM CIRCULAR NO. 35-A SERIES OF 1996, September 17, 1996 ]
AMENDMENTS TO MEMORANDUM CIRCULAR NO. 35, SERIES OF 1996
Further to Memorandum Circular No. 35, Series of 1996, Guidelines in securing the POEA Certified Photocopy of the Artist Record Book, the following are hereby amended:
1. Inclusion of the birthdate and skills category of the performing artist in the joint-letter request of the licensed agency and performing artist. In addition, the Booking Confirmation shall be attached to the request which shall contain the following information:
a. Name of Licensed Agency
b. Name of Authorized Training Agency
c. Name and composition of the Performing Artist Group
d. Name/s of the Performing Artist/s
e. Name of Performance Venue
f. Name of Foreign Principal’s Company; its President/Owner; its POEA Accreditation No.
g. Signatures of the Licensed Agency President/General Manager, Foreign Principal and the Artist/s
2. The pages of the ARB containing the name, address, birthdate, skills category and picture of the performing artist, as well as the signature of the POEA official who approved the issuance of the ARB shall be the photocopied.
This Order is effective immediately.
