(NAR) VOL. IV NO. 1 / JANUARY - APRIL 1993

[ PVAO SUPPLEMENTARY GUIDELINES NO. 90-2, June 12, 1990 ]

ASSIGNMENT OF CLAIM NUMBERS



1. Docket Books will be opened by the Administrative Division and by each Field Team.

2. Applicants in the different regions will submit their applications Teams concerned in accordance with procedures prescribed in Memorandum Circular No. 90-1.  In cases where an individual applicant, Accredited Conta Officer or PVAO Contact Officer in any region finds it more convenient, he m submit the application by mail or by hand direct to the Administrative Divis PVAO, Manila.

3.      Each application will be entered in the docket book and assigned a c number consisting of thirteen characters (letters and numerals) arranged in grouping:  AA-A00-00-000000.

3.1.   The first two characters indicate the type of benefit being claimed:

AP
Additional Pensions for Dependents of Totally Disabled Veterans

BA
Burial Assistance

DP
Death Pension for Indigent Parents DS — Disability Pension

DW
Death Pension for Surviving Spouse and Minor Orphans

EB
Educational Benefit

OV
Old Age Pension for Veteran

OW
Old Age Pension for Surviving Spouse

3.2.      The next three characters/numerals indicate the docketing office:

Manila Central Office, PVAO

FO1 
Field Team Number 1, Vigan

FO2
Field Team Number 2, Bayombong

FO3
Field Team Number 3, Dagupan

Field Team Number 5, Legaspi

3.3. The next two numerals indicate the year the application is filed: 90 fo year 1990, 91 for the year 1991.

3.4. The last six numerals indicate the sequence number of the application a received in the particular docketing office: from 000001 to 999999.
