ISSUANCE OF NEW ACCREDITATION ID CARDS WITH THE NEW ACCREDITATION NUMBER CONVENTION



In line with the thrust of the Corporation to enhance PhilHealth benefits for accredited professionals, PhilHealth will be issuing a new accreditation ID card which reflects the new accreditation number convention. The new number convention is designed to accommodate modifications and enhancements, for future benefit packages which is presently being developed by the Corporation. The new coding scheme will also accommodate the preferential payment for quality services that PhilHealth deemed provided by qualified professional providers. Further, the said new number convention is composed of twelve (12) numerical characters, to be used in all PhilHealth transactions starting January 1, 2006.

The first four (4) numbers shall be interpreted as stated in "ANNEX A.*"

The new accreditation ID cards will be released to professionals once every three (3) years after initial accreditation, re-accreditation upgrading of accreditation and during the renewal of accreditation. A physician who applies for upgrading of accreditation while his/her accreditation is still active has to submit all documentary requirements and should pay the accreditation fee of P1,500. A new ID card will be released to the concerned physician, and the validity of his/her accreditation is three (3) years from the date of issuance.

The information reflected in the accreditation ID card shall be as follows:

In case of damage or loss of accreditation ID card, the concerned professional can request for replacement of the same from the PhilHealth Regional Office, PhilHealth Service Office or Central Office, Accreditation Department. The requirements are as follows:

    1. Affidavit of loss, as the case may be.
    2. 1 x 1 ID picture (2 pcs.)
    3. One hundred and fifty pesos (Php 150.00) replacement fee.
