NEW CONTRIBUTION PAYMENT RETURN FORM FOR INDIVIDUALLY PAYING MEMBERS CONSIDERED AUTOMATIC MEMBERS OF THE NHIP.
Effective immediately, a new Contribution Payment Return Form for Individually Paying Members will be made available for use of all self-employed and voluntary members of the Social Security System (SSS) who are considered automatic members of the National Health Insurance Program (NHIP).
Dubbed MI-5, the new form is intended ot relieve the individually paying members from the burden of providing information being required by the existing M-5 Form from other membership categories. The new form will also facilitate and accelerate the posting of member's remittances since it is easily recognizable, thereby significantly reducing the time needed to classify forms according to membership categories.
