[ PHILHEALTH CIRCULAR NO. 71, S. 1999, August 31, 1999 ]

PROOF OF CONTRIBUTIONS FOR THE AVAILMENT OF MEDICARE BENEFITS



Please be informed that effective immediately, the following documents/PhilHealth remittances forms may be used as proof of contributions in the availment of Medicare benefits:
Membership Category
PhilHealth Remittance Forms
Currently Employed
Duly accomplished PhilHealth Form 1 is sufficient.
 
 

However, in case item no. 22 (certification of contribution) in PhilHealth Form 1 is not properly accomplished, submit the following:

* Copy of Employee Remittance List (RF-1) with duly validated Contribution Payment Return Form (M-5) and/ or receipt/s

Self-employed and Voluntary Members of the SSS (enrolled *PhilHealth Bank Receipts (PBRs) prior to July 1, 1999)   Any or a combination of the following:
 

*Duly validated Contribution Payment Return Form (M-5 or M1-5)
*Machine-printed receipt/s
*Receipt/s of PhilHealth-accredited banks
*Official receipt/s of the Republic of the Philippines(for over-the-counter payments)

Note: The above forms may be used together with the other proofs of contributions enumerated under PhilHealth Circulars 46, 46a and 46b, provided that the three (3) monthly contributions within the six (6) month period prior to confinement is satisfied.

Other provisions of Circulars 46, 46a and 46b that are consistent with this Circular are still in full force and effect.
