CHANGE OF HOSPITAL NAME



Pursuant to Section 55 b of the Implementing Rules and Regulations of the National Health Insurance Act (RA 7875) as amended by RA 9241, all hospitals shall comply at all times with the provisions of Republic Act 4226 or the Hospital Licensure Act and its prevailing Implementing Rules and Regulations as well as other applicable administrative issuances.

In this regard, Sections 16.6 and 16.7 of AO 147, s-2004 state that "for any change in name of the hospital, the concerned facility shall notify in writing within 15 days from the date of change, the Bureau of Health Facilities and Services of the Department of Health". The Corporation shall be furnished a copy of such notification.

The information in the corporation's database on the said hospital shall only be changed after its submission to the Corporation of the new DOH license-to-operate bearing the new name of the hospital and after the approval by the Accreditation Committee.

This circular takes effect fifteen (15) days after publication.
