[ PHILPOST ADMINISTRATIVE ORDER NO. 04-21, September 30, 2004 ]

REQUIREMENTS IN SECURING A POSTAL ID, AMENDMENT TO –



The requirements in securing a Postal ID are hereby amended to –

    1. Establish integrity of the PID.
    2. Avoid irregularities in the issuance of the Postal ID.
    3. Cast away doubts on the manner the affidavit of the witnesses are made and/or secured.
    4. Ensure authenticity of identity of the applicant.

Effective immediately the affidavit of the witness as to the identity of the applicant as required under Administrative Order No. 91-02, shall be dispensed with. Instead the following documents shall be required to identify the applicant.

    1. Certification of the Barangay Chairman that the applicant is a bona fide resident of the area.

    And any of the following documents:

    2. NBI Clearance

    3. Driver's License

    4. Senior Citizen's Card (with picture)

All other provisions pertinent to the issuance and fee of the Postal ID not in conflict herewith shall continue to be observed.
