USE OF THE REVISED APPLICATION FOR DRIVER’S LICENSE (ADL) FORMS
In line with the continuing implementation of the LTO Quality Management System (QMS)-ISO-2001-2008, effective 15 April 2010, all LTO Licensing Centers, District and Extension Offices with licensing functions are hereby directed to use the revised LTO Form No. 21 “Application for Driver’s License” (ADL).
In order to achieve a uniform implementation thereof, old ADL Forms must be returned immediately to concerned Regional Offices for proper disposal. Accordingly, the Chief, Property Section must see to it that sufficient supplies of the subject ADL forms are timely distributed to LTO field agencies.
