NEW PHILHEALTH IDENTIFICATION CARD FOR SPONSORED MEMBERS



Effective October 1, 2010, members under the Sponsored Program will be using the PhilHealth Identification Card (ID) in lieu of the “Family Health Card”. The new PhilHealth ID is rendered in the same design as the PhilHealth Number Card earlier issued to members in the employed Sector and Individually Paying Program. It will now serve as the sponsored member’s proof of membership registration and shall bear the following details:

- Permanent PhilHealth Identification Number (PIN)
- Complete name
- Signature
- 1” x 1” photograph
- Effectivity Date pertaining to validity period of coverage

The new PhilHealth ID card (Annex A*) has the following features: 

Front

- The PhilHealth corporate signature is situated at the upper left-hand portion of the card, above the box provided for the member’s 1” x 1” photograph
- A magnified representation of the PhilHealth icon rendered in watermark
- Yellow and green fine, curved lines set as background

Back

- Certification as to identity and entitlement of the cardholder and his legal dependents to PhilHealth benefits
- Signature of the PhilHealth President and CEO
- Official seal of the Republic of the Philippines rendered as watermark

All previously-issued Family Health Cards shall still be honored as proof of eligibility in the availment of PhilHealth benefits of sponsored members and their qualified legal dependents, provided that availment is within the validity period indicated in the card.
