AUTHORITY FOR LTO SAN CARLOS CITY NEGROS OCCIDENTAL DISTRICT OFFICE TO OPERATE UNDER THE LEAP PROGRAM



<DIV ALIGN=JUSTIFY>Date Filed: 11 June 2009

Pursuant to Memorandum Circular No. 2003-481 dated 30 September 2003 relative to the decentralization of post evaluation of driver &rsquo;s license applications and production of card type driver &rsquo;s license in some designated LTO Licensing Centers nationwide, the <U><B>LTO San Carlos City District Office</B> is hereby authorized to operate under the LTO Enhancement Activity Project (LEAP)</U>.

It is hereby emphasized that the following licensing transactions are covered by

this order:

a. All new driver &rsquo;s license/Conductor&rsquo;s license applications;

b. Renewal of driver &rsquo;s license subject for re-examination;

c. Application for additional restriction code/s;

d. Application for change status;  

e. Application for change name birth year/date; and

f. Issuance of student driving permit

As stated in the abovementioned Memorandum Circular (attached hereto<SUP STYLE="COLOR: RGB(255, 0, 0);">*</SUP>), all concerned officials and employees are directed to strictly comply with the licensing rules and regulations and to discharge their duties in accordance with the highest standards of public service.

This Order shall take effect on <U>15 June 2009</U>.

All previous Orders/Memoranda inconsistent herewith are hereby deemed superseded.
