3.    Secondary Cooperative   —   500.00
4.    Tertiary Cooperative   —   2,000.00
5.    Amendments

i.     Compliance with R.A. 6938    —   No Fee
ii.     Others —   P200.00

b. Docket Fees

1.    Mediation/Conciliation —   No Fee
2.    Merger/Consolidation/Division/
       Dissolution/Liquidation     —   P300.00

c.  Certification Fee — for any kind or type or certification issued by the Authority — P50.00

d. i. Certified true copies of documents — P25.00
