WHEREAS, Sec. 17 (d) Art. IV of R.A. 4419, known as the “Philippine Dental Act of 1965”, requires an applicant for the Board Licensure Examination for Dentists to submit College Diploma and Transcript of Records in support of the qualification as holder of Doctor of Dental Medicine (DMD);

WHEREAS, it has been established that some of the foregoing documents do not show the correct names of their schools/colleges/ universities and do not prove that their graduation is in order;

WHEREAS, in order to admit those applicants whose college Diplomas and Transcripts of Records are proofs of their completion for graduation, there is a need for submission of documentary evidence that will certify for the correctness of the name of the school/college/university and the evaluation, orientation, and validity of such graduation; and

WHEREAS, this requirement will guide the Board of Dentistry in admitting applicants for the examination who have duly met the academic course/degree qualification, and will insure that the data in their Diploma and Transcript of Records, among other things, include the correct name of their schools/colleges/universities whose course/degree duly recognized by the Government;

NOW, THEREFORE, the Board RESOLVES, as it is hereby RESOLVED, that applicants for the Board Licensure Examination for Dentists shall submit, in addition to their College Diplomas and Transcript of Records, a CERTIFICATE OF VALIDATION OF GRADUATION ISSUED BY THEIR RESPECTIVE DEANS INDICATING THEREIN THE CORRECT NAME OF THE SCHOOLS/COLLEGES/UNIVERSITIES, AND OTHER RELEVANT DATA.

RESOLVED, FURTHER, that this Resolution shall take effect after fifteen (15) days following its full and complete publication in the Official Gazette or in a newspaper of general circulation in the Philippines.

RESOLVED, FURTHERMORE, this requirements shall be imposed in the June 2007 and subsequent Board of Licensure Examinations.

RESOLVED, FURTHERMORE, a copy hereof shall be circularized to all schools, colleges, and universities offering the course of Dentistry for dissemination and information to all concerned.
