[ LTO MEMORANDUM, September 28, 2004 ]
PRESCRIBED FORMAT OF THE AFFIDAVIT OF LOSS OF LICENSE PLATES
It has been noted that many duplicate plates have been issued by this Office upon submission of Affidavit of Loss with misrepresentation of facts and/or signed by parties with questionable authority to represent or bind the registered owner of the covered unit. Based from reliable information, the regular plates subject of the affidavit are not actually lost and continuously are being used and attached to the assigned motor vehicle. As such, the issued duplicate plates are instead misused in the operation of other motor vehicles, which are either unregistered or engaged in colorum operations.
Accordingly, no application for duplicate plate shall be approved unless the supporting Affidavit of Loss is executed by its registered owner or by the President or General Manager in case the owner is a partnership or corporation. The sample of the prescribed format* of the Affidavit of Loss with provision for perjury is hereto attached for ready reference.

4. The said lost plate/s was/were never lent to another person or entity nor confiscated by any other law enforcement agency;
5. I bind myself to surrender the said plates to the district office that issued the same in the event that said plates are recovered;
6. If the DOTC thru the LTO/LTFRB, after inquiry, has found me responsible for the unauthorized use of the herein reported lost plates, I hereby signify my conformity to be meted the administrative fines and penalties due and imposed thereon, including the grounding of the unit/s covered by the Certificate of Public Convenience issued therefor, with waiver and desistance on my part for any claim or action by reason of the aforesaid punitive measures taken against me.
7. Purpose of this Affidavit of Loss:
8. I am executing this affidavit for the aforesaid purpose/s as well as for attesting to the truth of the statements herein declared, subject to the rules on perjury.
