[ POEA MEMORANDUM CIRCULAR NO. 12, s. of 1998, September 08, 1998 ]
REVALIDATION OF PDOS CERTIFICATES
In view of a number of incidents relative to the submission of fake/tampered and/or forged PDOS certificates of attendance and the Administration deems it proper to strictly monitor the issuance of said document to ensure its integrity.
Hence, all licensed agencies with accredited in-house PDOS programs and all accredited providers of PDOS, are hereby enjoined to submit samples of their certificates of attendance to the Director, Welfare and Employment Office, for revalidation.
The said certificate should reflect the following:
a. Name and address of the agency/PDOS entity and name of the principal.
b. Date of PDOS
c. Control number
d. Name and position of the authorized agency official/signatories and name of the PDOS trainor, with their corresponding signatures.
All parties concerned are requested to comply with this memorandum circular immediately.
