LOSS OF DDL NO. (S-2)



Pursuant to the powers vested in the Dangerous Drugs Board under Section 36 (a), Article VIII of Republic Act No. 6425, as amended, the following rules and regulations relative to the loss of Dangerous Drug License Number (S-2) by practitioners and their liability therefor, are hereby promulgated:

SECTION 1.       It shall be the duty of every physician, veterinarian, dentist or other practitioner to whom an S-2 License has been issued by the Dangerous Drugs Board (DDB) to immediately report in writing any loss thereof to the Board within TWENTY FOUR (24) to FORTY EIGHT (48) hours from the time it was lost, stating therein the circumstances surrounding the loss.

SECTION 2.       The concerned practitioner shall cause the publication of said loss in a newspaper of general circulation, and the expenses shall be borne by him.

SECTION 3.       Before a new S-2 License shall be issued by the Board, the practitioner shall be required to present to the Board proof of publication of the loss.

SECTION 4.       In case of failure to present proof of publication, the practitioner shall not be issued a new S-2 License. In addition, an administrative penalty shall be imposed upon him.

SECTION 5.       This Regulation shall take effect upon its publication once a week for two (2) consecutive weeks in a newspaper of general circulation.
