[ MARINA CIRCULAR NO. 2009-17, July 19, 2009 ]

NOTIFICATION ON THE CHANGE OF PRINCIPAL OFFICE ADDRESS



Pursuant to the provisions of Presidential Decree No. 474, Executive Order No. 125, as amended, and Republic Act (R.A.) No. 9295 and its Implementing Rules and Regulations, all shipowners/operators/entities and others concerned, accredited as a MARINA enterprise under MARINA Circular Nos. 108, 181, 165, 186, 174, 2006-003 and 2007-02 shall be required to notify this Authority within 15 calendar days on the change of their principal office address.

Failure to notify this Authority shall subject the accredited enterprise to the following fines and penalties, and without prejudice to the suspension or cancellation of their accreditation with this Authority.

1st offense - P50,000.00
2ndoffense - 75,000.00
3rdoffense - 100,000.00

This Circular shall take effect fifteen (15) calendar days after its publication once in a newspaper of general circulation.
