OFFICIAL RECEIPT OF MEDICAL TEST AND CLEARANCES



One of the issues raised in the Corrective and Preventive Action Request (CPAR) Forms submitted by the regional offices is the resistance of the Driver License (DL) applicant to submit the original copy of the Official Receipt (OR) of the medical test and the original copies of the required clearances. They consider these documents as properties of the DL applicants. During the 2nd  Process Control Team (PCT) briefing, it was discussed that the xeroxed copies of the OR and Clearances can be accepted as long as the original copies of the said documents are presented by the DL applicant. The evaluator shall record/note down that the original copies were presented.

For uniformity of implementation, the aforesaid agreement during the 2nd PCT briefing shall be adopted and implemented in all DL sites.
