SUBMISSION OF TRANSCRIPT OF RECORDS



WHEREAS completion of a degree or course is evidenced by the transcript of records, diploma, and special order;

WHEREAS two (2) of the said documents are at least required for submission together with the application for examination;

WHEREAS the non-submission of any of the two (2) documents is a cause for deferment of the registration of a successful examinee or of an applicant therefor without examination;

WHEREAS the submission of the certified true copy of the transcript of records can substantially establish compliance with the academic and non-academic requirement provided that it bears likewise the degree obtained, date of graduation, and special order number unless it is not required in view of accreditation of the course by the DECS or of being a graduate of a state school/college; and

WHEREAS the limitation to one vital document can promote expeditious processing of application and lessen inconveniences.

NOW, THEREFORE, by virtue of Sec. 5 (b) and (n) of P.D. No. 223, the Commission hereby RESOLVED, as it so RESOLVES, to prescribe and issue the herewith guidelines, to wit, effective April, 1994:

1.         Every examinee shall submit with his/her application for examination a certified true copy of a transcript of records containing these vital data: (a) degree or course, (b) date of graduation, and (c) special order number unless exempt therefrom due to accreditation of a course by the DECS or to being a state school/college graduate.

2.         An applicant for registration without examination shall likewise comply with the foregoing requirement.

FURTHER, RESOLVED, this Resolution shall upon approval by the Commission, be effective after fifteen (15) days following completion of its publication in the Official Gazette or any newspaper of general circulation.

FINALLY, RESOLVED, this Resolution shall be widely circularized or disseminated to all concerned through the schools/colleges with courses requiring licensure examination or registration without licensure examination.
