SUBMISSION OF QUARTERLY PERFORMANCE EVALUATION OF LTO ACCREDITED PHYSICIANS
Pursuant to Memorandum Circular No. RIB-2007-889 establishing the criteria in the evaluation of accredited physician's performance in compliance to the requirement of ISO 9000-2001 and for effective and efficient public service, the performance evaluation should be submitted by the Regional Committee on Accreditation of Physicians to the Central Office's Committee Chairman on Accreditation of Physicians quarterly for each LTO-accredited physician in their area.
Furthermore, all ocular inspections done by the Committee on Accreditation of Physicians to new applicants for Accreditation of Physicians should be in a narrative form in compliance to ISO 9000-2001.
All memoranda/orders/issuances that are inconsistent herewith are deemed revoke and/or superseded.
