[ BAI MEMORANDUM CIRCULAR NO. 02, 2010, October 05, 2010 ]

GUIDELINES ON THE ADOPTION OF THE STANDARD FORMS OF THE CERTIFICATE OF VACCINATION, THE DOG TAGGING SYSTEM, MANDATORY REGISTRATION AND CENTRAL DATABASE FORMS FOR USE OF THE LOCAL GOVERNMENT UNITS AS PER PROVISIONS OF THE ANTI-RABIES ACT OF 2007 (RA 9284)



The following guidelines on use of the standard forms of the certificate of vaccination, dog registration, central database and the dog tagging system shall be adapted and implemented at all levels of the Local Government Units (LGUs).

I. MANDATORY REGISTRATION OF DOGS

As stipulated in Sec. 4, Rule 4.2.2 of the Law, a central database system shall be established and be required to all LGUs. The registry of all dogs for vaccination using the prescribed form is a requisite. (See Annex A*)

A permanent and non transferable registry shall be provided by the LGUs for all registered dogs and shall be renewed annually. Dog tag/s shall be issued to owner upon registration of his/her dog/s.

II. IDENTIFICATION OF VACCINATED ANIMALS

A Standard vaccination tagging system ( registration number, code bearing the name of the city, municipality and barangay) shall be used for identification of vaccinated animals. These tags shall be distinguishable in shape and color. The dog tag shall be made of anodized aluminum and should not be less than 1 x 1 inches and 1.5 mm in thickness. The dog tags shall be distinguished by its color specified for the year as shown below.

III. CERTIFICATE OF VACCINATION

A Standard certificate of vaccination shall be used by all veterinarians in the LGUs. The certificate shall be issued to the owner upon vaccination of his/her dog/s and shall be enclosed in the registry form of the specific dog registration. All dogs must be registered to be vaccinated. It is recommended that the size of the vaccination certificates be 4 ¼ inches x 5 ½ inches ( ¼ of letter size paper) and shall contain the following information. (see Annex B)

This Memorandum Circular shall take effect immediately.
