CERTIFICATE OF CONFIRMATION



It has been observed that some district offices are accepting Certificates of Confirmations issued by Regional Offices other than their own or the Central Office Registration Section on MAID Reports. Everyone concerned is advised to refer and comply strictly with the provisions of MC #90-119, 33 and 83MC-DIR-017 and Circular 84C-DIR-013 governing the issuance of CCs.

In this connection therefore, only CCs issued by the Central Office Registration Section and the Regional Office having jurisdiction of the registering District or Extension Office shall be accepted. In particular, LTO Regional Office No. III shall cease to issue its own LTO Form No. 6 (CCs) in order not to confuse the public and effect the control and monitoring system already in place.

Collection of fees for the CCs shall be collected at the district office during registration using the same receipt (MVRR/MVMRR).

All previous memoranda, order or instruction contrary herewith are hereby superseded.
