Santos Ave., Upper Bicutan, Taguig City.
You are further notified that petitioner Mayleen Ting who was born on June 04, 1977 in Portland, Oregon, United States of America, arrived at the Manila Airport in the Philippines sometime in 1978 and since then resided at No. 27 Tanguile Road, Forbes Park, Makati City until she moved to her new current address at South 19-A Pacific Plaza Towers Condominium, Bonifacio Global City, Taguig where she has been residing for at least 1 year. She intends to present as witnesses Mr. Ernesto Rufino of 19 Banaba Road, Forbes Park, Makati City, and Mr. Peter Pua of 9629 Pililia St., Brgy. Valenzuela, Makati City.
