AMENDMENT TO ADMINISTRATIVE ORDER NO. 1, S. 2003 DATED JANUARY 2, 2003 RE: OPERATIONAL GUIDELINES IN THE CONDUCT OF PRE-EMPLOYMENT MEDICAL EXAMINATION OF OVERSEAS WORKERS AND SEAFARERS



Administrative Order No. 1 series of 2003 dated January 2, 2003 is hereby amended to include the following information under provision III.2. Specific Guidelines in the filing of X-ray films: Name of Client/Patient, Age, Sex, Date and Name of Clinic/Hospital in the filing of x-ray film.

This Order shall take effect immediately.
