Upon thorough evaluation of the proposals of the Truck-for-Hire Service operators, the Board hereby exempts Truck-for-Hire Service from the coverage of Memorandum Circular No.2004-004 (Re: Amending Memorandum Circular Nos. 99-011, as Amended, and 2001-001 (Implementing Guidelines of the Revitalized LTFRB Passenger Personal Accident Insurance Program [PPAIP] for Public Land Transportation Services), provided that the following requirements are submitted: (1) List of Drivers and Riders and their Employment Contracts; and (2) Copy of the Certificate of Cover of their personal accident insurance.

This Memorandum Circular supersedes any and all issuances inconsistent herewith, and take effect immediately following the filing of three (3) copies hereof with the UP Law Center, pursuant to Presidential Memorandum Circular No.11, dated 09 October 1992.
