1. Inactive members refer to existing members of the National Health Insurance Program (NHIP) who have not paid premium contributions for the past twelve (12) months.
2. RegistrationExisting members and new enrollees who are professionals/other professionals included in the Updated List of Professionals/Other Professionals hereto attached and made an integral part hereof as Annex A* under the Individually Paying Program (IPP) shall:2.1 Register or update their member record by accomplishing the PhilHealth Member Registration Form (PMRF) to reflect their profession and estimated monthly family income for the past twelve (12) months:
2.2 Submit a photocopy of their Professional Regulation Commission (PRC) ID card for professionals or the latest Integrated Bar of the Philippines (IBP) Receipt for lawyers or a photocopy of the latest local Business Permit for other professionals;
2.3 Submit a photocopy of the latest Income Tax Return (ITR) if the declared monthly family income for the past twelve (12) months is Twenty-Five Thousand (P25,000) and below. In case the existing member or new enrollee does not have an ITR, a photocopy of the Community Tax Certificate (CTC) for the current year or any other documents that would show proof of family
income shall be submitted.
3. Premium Contribution
3.1 New enrollees who are professionals/other professionals and unable to show proof that their estimated monthly family income is P25,000 and below shall pay the new premium contributions of SIX HUNDRED PESOS (P600) per quarter or ONE THOUSAND TWO HUNDRED PESOS (P1,200) per semester or TWO THOUSAND FOUR HUNDRED PESOS (P2,400) per year effective immediately. 3.2 Existing members and new enrollees who are professionals/other professionals shall pay NINE HUNDRED PESOS (P900) per quarter or ONE THOUSAND EIGHT HUNDRED PESOS (P1,800) per semester or THREE THOUSAND SIX HUNDRED PESOS (P3,600) per year effective January 1, 2012.
