(NAR) VOL. 25 NO. 1 / JANUARY - MARCH 2014
Pursuant to the provisions of Republic Act No. 7925, Act No. 3846, as amended, Executive Order No. 546 and other applicable laws, rules and regulations, the Commission hereby promulgates the following additional guidelines in the purchase of SRRS equipment:
A. Qualifications of applicants 1. For Individuals:
a. Applicant must be Filipino citizen b. Must be 18 years old and above 2. For companies/corporations a. Must be duly registered with the Department of Trade and Industry (DTI) or with the Securities and Exchange Commission (SEC) B. Documents to be submitted for application for Permit to Purchase 1. Letter of Intent 2. Duly accomplished (in triplicate) and notarized application form for Permit to Purchase SRRS equipment 3. Duly accomplished and notarized Information Sheet 4. Photocopy of any valid Identification Card, original to be presented (any government issued ID, Company ID, School ID, or any document showing proof of identification) 5. Certified copy of SEC/DTI registration certificates (for corporation or partnerships) 6. For security agencies, updated copy of the valid license to operate issued by the Philippine National Police (PNP) 7. For corporation or partnership, copy of the Corporate Secretary’s Certificate authorizing the applicants to purchase SRRS equipment for the company any valid identification card as provided in par (4) C. NTC Fees Applicants of Permit to Purchase (SRRS equipment) shall pay upon filing of the application the following fees: 1. Application fee (Php 180/unit) 2. Permit to Purchase (Php 96.00 /unit) 3. Documentary Stamp Tax (Php 15.00/document)
This Circular shall take effect fifteen (15) days after publication and may be revised, revoked or amended, in the interest of public services and as determined by the Commission.
Any circular, order or memorandum, which is inconsistent herewith, is deemed superseded.
